The law protects the relationship between a client and therapist and information cannot be disclosed without written permission. All client information is confidential and will not be released to any third party except at the specific written request of the client. This includes release of the information that a person is, or is not, a client. The only exception to this policy would occur if nondisclosure would directly jeopardize the life or safety of the client or others.
- Suspected child abuse or dependent adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
- If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety.
I understand that a client has a right to self-determination in the continuation of treatment and/or the acceptance of referrals. I am ethically and personally committed to my clients’ well-being and would welcome discussion of any appearances to the contrary.
